Association of Community Health Plans (ACHP)

The Alliance of Community Health Plans (ACHP) is the national trade association for America’s nonprofit, community-based member health companies. These health plans collaborate with their provider partners to deliver higher-quality coverage and care to tens of millions of Americans across nearly 40 states and DC.
Goals

Launch an association-wide Chronic Disease Pledge to track and improve health outcomes in diabetes, hypertension and childhood obesity by 2030.

Improve health equity and address social drivers of health such as food insecurity, housing and transportation.

Secure Chronic Disease Pledges from the entire ACHP membership.

Help ACHP members get and stay on track to achieve their Chronic Disease Pledges.
Approach

Provided ACHP members with technical assistance (TA) and support to onboard them into the Pledge and document their specific Pledges.

Communicated the value-proposition of the Chronic Disease Pledge with all ACHP members and the ways in which it will improve health in communities and improve health equity.

Planned and launched a sophisticated year-long TA program comprised of Cohort Meetings, subject matter experts, tools and resources, research articles with strategies for improving chronic conditions, and case studies with winning approaches to patient engagement and disparities reduction
Outcomes:

100% of ACHP Founding Members committed to the Chronic Disease Pledge.

6 member plans committed to a Pledge with more than one condition.

70% of member plans, on average, attend Cohort meetings and participate actively, sharing strategies, learnings and resources with each other.

Uncovered and documented successful strategies from Member plans and identified implications for national health policy.

Member plans achieved early wins by sharing data and improving in chronic disease outcomes.
